STAGE REQUIREMENTS FORM

To assist the product crew in setting up the stage for you as quickly and efficiently as possible, all performers are required to print this page, fill it out and send it to us. If any of your performances involve a dramatically different line-up, please print this page many times or photocopy and fill out a form for each line-up.

PLEASE NOTE: This form will be forwarded to the Venue Manager and Sound Engineer of each performance venue.
